An-Pyng Sun (Social Work), together with Ariann Chelli (Desert Hope Treatment Center in 51ԹApp) and Maureen Strohm (HCA Sunrise Health GME Consortium at Southern Hills Hospital in 51ԹApp) wrote an article, "Connecting Hospitalized Patients with Substance Use Disorder to Community Continuing Coordinated Care: The Critical Role of Hospital Social Workers,” which appeared in the National Association of Social Workers (NASW) Alcohol, Tobacco, and Other Drugs Specialty Practice Section 2025 Spring/Summer newsletter. One in every seven hospitalized patients has an active substance use disorder (SUD). Hospital often can only address the patient’s acute SUD symptoms despite SUD being a chronic disease that requires community continuing coordinated care. This article discusses the critical role of hospital social workers in ensuring a successful transition from hospital to community.
